Go to that link, reply with "Start". This will put you at level 1 and give you the large avatar. If you fall below 75% during this, you will lose your large avatar.

For level 2 you need to stay at 100% for 28 consecutive days, and reply with "Complete" on day 28. At that time you will retain your large avatar unless you fall below 20%.
