i just got back from the dentist... if you had asked me 1h ago id say that was the fuckin worst pain ever.
Real worst pain must have been falling from the 2nd floor of a barn on a sandbox, i was like 4-5 years old and i came back in the restaurant holding a piece of leg, it was at least 1cm thick and bleed like hell.
2nd one prob is when i got hit by a car (few years ago) and fell on the street, slided for about 3-5 meters. If i pushed my skin softly you could see the bone that was now a few milimeters thinner. that DID hurt (where your belly button is but on the left of your body)

Anyway, worst pain ever.... no idea. I think once pain gets too bad you pass out from it, or most people do, so if you ever passed out from pain; the seconds right before that?

ive had quite a few once when i was little my dad hung me on the rim of my basketball hoop, unfortunetly it ended up that the rim comes down on and hits the bak of my head really hard i was knocked unconsious and i was sent to the emergency room and i had to get stitches in the bak of my head i got major blood loss
another time was when i was on my bike goin down these huge hills goin 40/45 and then i hit this huge rock and i fell offf my bike and i tumbled down these woods getting hit by like every tree it hurt like hell, once agian i was sent to the hospital where i stayed for recovery for 3 weeks i pretty much had NO skin on my legs/arms/face
when i was eight i was running up to my house, tripped over on of our flagstones on the path and slammed my head on the concrete step. I had concussion, chipped skull and severe blood loss thats worst for me. Don't remeber to much though because i was unconcious after about a minute lol
